We support patients with preventative health measures that can help reduce the risk of chronic disease impacting your\n                      life.<\/p>\n                    <p>We\u2019re experienced in working with patients to help formulate Chronic Disease Management Plans (GPCCMP) for patients who have complex conditions that require ongoing care in a structured way.<\/p>\n                    <p>A GPCCMP helps formalise a plan of action you have agreed with your GP, by identifying your health and care needs, setting out the services to be provided by your GP, and listing the actions you can take to help manage your condition.<\/p>\n                    <p>As part of the GPCCMP, patients will be eligible for up to 5 allied health visits per calendar year that Medicare will partially subsidise.<\/p>\n                    <p><strong style=\"font-weight: 700;\">Allied health disciplines that can accept GPCCMP\u2019s include;<\/strong><\/p>\n                    <ul>\n                      <li>Aboriginal health workers<\/li>\n                      <li>Audiologists<\/li>\n                      <li>Chiropractors<\/li>\n                      <li>Diabetes educators<\/li>\n                      <li>Dietitians<\/li>\n                      <li>Exercise physiologists<\/li>\n                      <li>Mental health workers<\/li>\n                      <li>Occupational therapists<\/li>\n                      <li>Osteopaths<\/li>\n                      <li>Physiotherapists<\/li>\n                      <li>Podiatrists<\/li>\n                      <li>Psychologists and<\/li>\n                      <li>Speech pathologists.<\/li>\n                    <\/ul>\n                    <p><strong style=\"font-weight: 700;\">GPCCMP services include:<\/strong><\/p>\n                    <ul>\n                      <li>Diabetes<\/li>\n                      <li>Heart failure<\/li>\n                      <li>Ischaemic heart disease<\/li>\n                      <li>Chronic kidney disease<\/li>\n                      <li>Hypertension \/ high blood pressure<\/li>\n                      <li>Hyperlipidaemia \/ high cholesterol<\/li>\n                      <li>Depression<\/li>\n                      <li>Asthma<\/li>\n                      <li>Chronic obstructive pulmonary disease<\/li>\n                      <li>Osteoarthritis<\/li>\n                      <li>Osteoporosis<\/li>\n                      <li>Autoimmune conditions<\/li>\n                    <\/ul>\n                    <p>\n                      <br>\n                    <\/p>\n                  <\/div>\n                <\/div>\n              <\/div>\n            <\/div>\n          <\/div>\n        <\/div>\n      <\/div>\n    <\/div>\n  <\/div>\n<\/div>","protected":false},"excerpt":{"rendered":"<p>CHRONIC DISEASE MANAGEMENT Our team of specialist GPs are passionate about identifying and helping patients treat chronic medical conditions.
