We regularly receive requests from third party providers such as Insurance companies, Lawyers, Workcover and TAC requesting release of a patient’s medical file.
You will always be asked to attend the clinic to read the request that has come from the third party so that you are aware of the information that they are requesting. This also requires signed consent by you before any information is sent.
You can also request our clinic to release information from your file to another clinic, service provider or to access your own personal file. This also requires signed consent by you before any information is sent.
Please note a charge may apply for files sent in paper format.
