This document authorises your appointed medical treatment decision maker to make medical decisions on your behalf. It becomes effective only in the event that you are deemed incapable of making such decisions yourself.
This form is to provide consent for your support person to access, or help you to access, health information relevant to your medical treatment.
This form does not provide your support person the power to make medical treatment decisions on your behalf.
An advance care directive is a legal document that outlines your preferences for future medical treatment and personal care, in case you are unable to make those decisions for yourself in the future . It can include specific instructions on treatments you do or do not want, as well as your values and goals for care.
